Responsibilities:
Provide direct patient care according to policies and procedures
Administer medications and treatments as prescribed by physicians
Monitor and report changes in patient condition to the nursing staff and physicians
Collaborate with interdisciplinary team to develop and implement individualized care plans
Maintain accurate and detailed records of patient care

Requirements:
Current LPN licensure in the State of Vermont
Ability to work effectively as part of a team
