Job Description

  • Provide concrete and counseling services to families with children referred by DHS.

  • Conduct client intake.

  • Develop Individual Living Plan (ILP).

  • Identify benefits or entitlements that may be available to the family.

  • Meet with the family as required, to evaluate progress and assist families in developing and meeting individual goals.

  • Provide necessary referrals for services, including mental health and or substance abuse counseling, as required.

  • Advocate on the family's behalf when necessary.

  • Coordinate with other faciilty staff including Housing Specialists, Security, and other Case Workers to ensure effective service delivery.

  • Maintain data collection and case records as required by the Funder.

  • Adhere to supervision sessions, and participate in in-service training programs and conferences.

  • Assist clients to regain and maintain control over their lives and over their own transition to independent living.

  • Facilitate the development of self-help skills, support systems, and coping strategies.

  • Ensure families adhere to the facility's rules and regulations and other duties as assigned by the Program Director.
