- Provide concrete and counseling services to families with children referred by DHS.
- Conduct client intake.
- Develop Individual Living Plan (ILP).
- Identify benefits or entitlements that may be available to the family.
- Meet with the family as required, to evaluate progress and assist families in developing and meeting individual goals.
- Provide necessary referrals for services, including mental health and or substance abuse counseling, as required.
- Advocate on the family's behalf when necessary.
- Coordinate with other faciilty staff including Housing Specialists, Security, and other Case Workers to ensure effective service delivery.
- Maintain data collection and case records as required by the Funder.
- Adhere to supervision sessions, and participate in in-service training programs and conferences.
- Assist clients to regain and maintain control over their lives and over their own transition to independent living.
- Facilitate the development of self-help skills, support systems, and coping strategies.
- Ensure families adhere to the facility's rules and regulations and other duties as assigned by the Program Director.
