The Mental Health Therapist provides counseling/therapy services for pediatrics, adults, and geriatric populations in an outpatient setting with a PCMH model. The Mental Health Therapist performs client and/or community focused resource coordination activities to ensure clients and/or community members receive comprehensive care. The Mental Health Therapist must also have background and knowledge of trauma and the effect it has on development. The Mental Health Therapist must possess training and group facilitation skills, knowledge of related community resources, and the ability to collaborate with other community organizations and agencies.
Please note: This position has the ability to become hybrid with a three-day on-site / two-day remote framework after the successful completion of the 6-month probationary period
Essential Duties
- Provide therapy, psychotherapy and/or counseling to an entire population
- Conduct intakes, psychosocial assessments and other risk assessments in accordance with program standards
- Develop counseling interventions in coordination with the client and/or their family
- Provide individual, family and group counseling
- Complete comprehensive assessments, treatment plans, progress notes and other documentation in a timely manner
- Complete written documentation to program standards
- Meet billing requirements as set by the agency
- Participate in weekly individual supervision as required
