Summary of Essential Functions
- Provides intensive, time-limited, community mental health case management to participants in the Permanent Supportive Housing program who are transitioning from hospitalization or homelessness to permanent housing within the Staunton/Waynesboro, Augusta, and Highland County catchment areas.
- Services include, but are not limited to: a minimum of a weekly in-person face-to-face contacts, referral to other agencies, supportive counseling, outreach, crisis intervention, and assessment.
- Collaborates with PSH Specialist to promote housing stability.
- Provides comprehensive initial and annual assessment of client strengths and needs.
- Participates in development of the consumer service plan.
- Refers, links, and coordinates Medicaid and non-Medicaid services.
- Provides supportive counseling and case management services.
- Provides crisis intervention and crisis stabilization services as needed.
Requirements
Minimum Requirements
- Bachelor's degree in human services or related field.
- Previous work or internship experience that provides the necessary knowledge, skills, and abilities.
- Must pass required pre-employment screenings, including, but not limited to, criminal history and experience verification.
- Current Virginia driver’s license with zero points or more.
