Speech Therapist (Home Health) 

In this role, you will serve as a vital partner in your patients’ care, helping them regain communication, swallowing function, and overall quality of life in the comfort of their own homes. You will work collaboratively with physicians, patients, and caregivers to develop and implement individualized plans of care that promote recovery, safety, and independence.

Your expertise will directly impact each patient’s ability to function at their highest potential through evaluation, treatment, education, and ongoing support.

Qualifications

  • Current Speech Therapist license (State of Arizona required) 
  • Minimum of two (2) years of Home Health experience preferred
  • Strong clinical judgment and ability to work independently
  • Commitment to the mission and values of Haven Home Health


Key Responsibilities

  • Perform initial evaluations and develop individualized treatment plans
  • Provide skilled speech therapy services as ordered by the physician
  • Assess, treat, and educate patients with communication and swallowing disorders
  • Collaborate with interdisciplinary team members to coordinate patient care
  • Participate in case conferences and care planning
  • Monitor patient progress and adjust treatment plans as needed
  • Complete timely and accurate clinical documentation, including OASIS assessments
  • Ensure compliance with agency policies and regulatory standards
