We are currently seeking a Support Services Coordinator to join our Support Services department. As a Support Services Coordinator, you’ll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. Key Responsibilities

  • Completes calls to assess eligibility of participants according to program-specific criteria and timeframes. Refers non-eligible participants to appropriate community resources.

  • Explains criteria and application process for applicable waiver programs, Medicaid Services and other community programs to callers in need of services.

  • Adheres to all HIPAA (Health Insurance Portability and Accountability Act) regulations to ensure confidentiality of both consumer and employee information.

  • Sends documents to consumers, home care team members, and callers with 100% accuracy to ensure HIPAA compliance; this includes all clinical documentation, waiver applications and instructions.

  • Investigates returned documents for correct addresses; updates consumer database and/or notifies case manager of needed changes or inability to obtain correct information; notifies provider management of changes needed in the provider database.

  • Emails accurate, complete information from callers to company employees as needed; accurately documents all calls in company electronic Caller Log per protocol.

  • Participates in, and occasionally leads, cross-training efforts to ensure adequate coverage for all administrative services tasks.



Minimum Qualifications


  • High School Graduate or GED equivalent, with knowledge of medical terminology.

  • At least one (1) year of experience in telephone customer service (multi-line, high call volume) and data entry preferred.

  • Ability to provide excellent customer service, including effective interpersonal, verbal and written communication skills, independently and as part of a team
