Job Summary

Provides psychosocial assessment, counseling, advocacy, and coordination of community resources for patients and families. Supports continuity of care, discharge planning, and emotional well-being in the hospital setting.

Minimum Qualifications

  • Bachelor’s degree in Social Work required
  • Licensed or license-eligible per state requirements
  • Basic Life Support (BLS) certification
  • Knowledge of community resources and discharge planning

Preferred Qualifications

  • Hospital or healthcare social work experience preferred

Essential Duties & Responsibilities

  • Conduct comprehensive psychosocial assessments for patients and families
  • Provide counseling, crisis intervention, and emotional support
  • Coordinate discharge planning and post-acute community resources
  • Collaborate with physicians, nurses, and interdisciplinary care teams
  • Advocate for patient rights and continuity of care
  • Maintain accurate, timely clinical documentation

Physical Requirements

  • Ability to stand and/or walk for extended periods.
  • Frequent bending, reaching, squatting, and kneeling.
  • Lift, carry, push, and pull up to 50 lbs independently; >50 lbs with assistance/devices.
  • Assist with patient repositioning and transfers using proper body mechanics and equipment.
  • Manual dexterity for clinical procedures and computer use; adequate vision, hearing, and speech.
