What You’ll Be Doing (High Level)
Providing skilled PT care in patients’ homes
Completing admissions and routine visits
Developing and carrying out individualized care plans
Collaborating with nurses, therapists, and providers Participating in interdisciplinary team meetings (The formal job description comes later—this is about the experience.)

What You’ll Need
Oregon PT license (or ability to obtain before start date)
1–2 years PT experience (home health preferred, not required)
Current CPR certification
New PT graduates encouraged to apply
