Overview

We are currently seeking a per-diem Social Worker to provide services in our Case Management Department. This role requires availability for alternating weekends, as well as coverage for staff absences, including holidays and vacations. 

Responsibilities

• Prioritize patient and family issues for short-term intervention and the need for referral for post-discharge care.
• Assist the case management and health care teams in developing an appropriate, safe discharge plan for complex cases.
• Identify and intervene with patients and families to remove barriers to effective care, patient autonomy, and timely discharge. 
• Provide expertise to the case management and health care teams in addressing issues impeding full patient/family participation in treatment and discharge, and providing information on community and discharge resources.
• Link patients/family to post-hospitalization services required for appropriate care.

Qualifications

• Master’s in social work from an accredited school of social work.
• Current license as an LMSW/LCSW.
• 1 year of prior experience required in medical social work or comparable experience.
• Knowledge of all rules and regulations of child abuse/neglect reporting as appropriate.
• Documented experience and knowledge of two direct practice modalities.
• Good interpersonal verbal/written communication skills.
• Knowledge of a second language is preferred and may be required for some positions.

Pay Range

USD $45.81 - USD $64.19 /Hr.
