Job Scope:
The duties and responsibilities of the Social Worker are to provide medically-related Social
Service programs to meet the needs of residents, determining the type of assistance required,
assisting in obtaining benefits from agencies and organizations such as Medicare, Medicaid, Social
Security, Veterans Benefits, Psychiatric, Psychology, Dental, Vision, and others.
Responsibilities:
• Assesses residents to identify the psychosocial, financial, medical, environmental, community,
and discharge needs. Document assessment, clinical team meeting minutes, and community resources
referrals.
• Uses effective interpersonal relations and communication skills; facilitates the use of these
skills
by other team members to achieve desirable outcomes.
• Attends Interdisciplinary team (IDT) meetings, participate in the patient care planning process,
and collaborate with the IDT to promote coordination of patient care.
• Coordinate effective patient discharges to the community. Communicate and document planned
discharge needs.
• Demonstrate commitment, professional growth, and competency by maintaining a working knowledge
of public and private eligibility standards and requirements.
• Accept clinical assignments that are consistent with education and competence to meet the needs
of the patients.
• Encourages, listens, respects, and reassures residents and families in all aspects of social
service.
• Participates in ongoing training and in-services for improving knowledge and updating
skills.
• Foster teamwork with co-workers, show initiative in overcoming obstacles, take actions that
assist others beyond job responsibilities, and have a positive impact on co-workers, residents, and
visitors.
• Seek out and perform other duties as assigned or needed.
Qualifications:
• Compassion for and desire to work with seniors!
• Minimum Bachelor's Degree or higher in Social Work from a state -accredited program or
bachelor's degree with one-year experience providing social services in a healthcare setting.
• 1-year social work experience in the healthcare field.
• Excellent interpersonal skills and working knowledge of community resources.
• Excellent communication skills and knowledge of various computer programs, including Microsoft
Office and web-based software systems.
