Job Description

We are seeking a dedicated Registered Nurse to join our Hospice team, where you’ll play a vital role in providing skilled, patient-centered care to through end-of-life care with dignity, comfort, and compassion.



Key Responsibilities




  • Provide skilled nursing care to hospice patients in their homes using the full nursing process.

  • Assess the physical, mental, emotional, and social needs of patients and families.

  • Develop, implement, and update individualized plans of care in coordination with the interdisciplinary hospice team.

  • Administer medications and treatments as prescribed and provide education on disease progression, symptom management, medications, and comfort care.

  • Serve as case manager for assigned patients, coordinating care with physicians, caregivers, team members, and community resources.

  • Support patients and families in coping with illness, loss, and end-of-life decisions.

  • Accurately and timely document assessments, interventions, and care provided in the electronic medical record.

  • Participate in interdisciplinary team meetings, quality improvement efforts, and the hospice on-call rotation.



Qualifications




  • Current Texas RN license or compact equivalent.

  • BLS for Healthcare Providers certification required.

  • Valid driver’s license, current auto insurance, and reliable transportation required.

  • Minimum of one year of RN experience preferred.

  • Prior hospice experience preferred.

  • Strong communication, organizational, interpersonal, and critical thinking skills.

  • Ability to provide compassionate care and emotional support to patients and families in a home-based care setting.
