We are seeking a dedicated Registered Nurse to join our Hospice team, where you’ll play a vital role in providing skilled, patient-centered care to through end-of-life care with dignity, comfort, and compassion.
Key Responsibilities
- Provide skilled nursing care to hospice patients in their homes using the full nursing process.
- Assess the physical, mental, emotional, and social needs of patients and families.
- Develop, implement, and update individualized plans of care in coordination with the interdisciplinary hospice team.
- Administer medications and treatments as prescribed and provide education on disease progression, symptom management, medications, and comfort care.
- Serve as case manager for assigned patients, coordinating care with physicians, caregivers, team members, and community resources.
- Support patients and families in coping with illness, loss, and end-of-life decisions.
- Accurately and timely document assessments, interventions, and care provided in the electronic medical record.
- Participate in interdisciplinary team meetings, quality improvement efforts, and the hospice on-call rotation.
Qualifications
- Current Texas RN license or compact equivalent.
- BLS for Healthcare Providers certification required.
- Valid driver’s license, current auto insurance, and reliable transportation required.
- Minimum of one year of RN experience preferred.
- Prior hospice experience preferred.
- Strong communication, organizational, interpersonal, and critical thinking skills.
- Ability to provide compassionate care and emotional support to patients and families in a home-based care setting.
