Description

Schedule: 32 hours per week Monday to Friday; rotating Saturday mornings

Work includes providing care at the four surrounding satellite clinics: Tilden, Elgin, Clearwater and Orchard as well AMH Family Practice in Neligh.

Reports To:  Chief Clinical Operations Officer

Requirements

  • Must be a graduate of an accredited School of Nursing and possess a current nursing license in the State of Nebraska.
  • Must possess a valid driver’s license and have an excellent driving record.
  • CPR certification required.
  • Clinical experience including clinic EHR experience preferred but not required.

Summary

Provides direct and indirect patient care in the clinic setting. Provides care that reflects initiative, flexibility and responsibility indicative of professional expectation with a minimum of supervision. Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system.  Communicates with outpatient clinic physicians about changes in patient's status, symptomatology and results of diagnostic studies.  Is able to respond quickly and accurately to changes in condition or response to treatment.

Essential Functions:

1.  Assess, plan, implement and evaluate patient care needs. This includes assessing patient’s condition, triaging patient phone calls, counseling patients, providing patient education, taking patient histories, administering prescribed medications, changing dressings, cleaning wounds and monitoring patient vital signs.

2. Documents patient care in the healthcare record according to established standards. 

3. Ensures own professional effectiveness through education and professional development. 

4.   Participates in the development of and maintains established policies, procedures and objectives; quality improvement; safety; environmental and infection prevention programs. 

5.    Performs other duties as assigned.
