Greenfield Rehabilitation Agency is excited to announce an opportunity for a Certified Occupational Therapy Assistant (COTA) to join our therapy team at our newly acquired senior community in Sauk City, WI.

This PRN (as-needed) role offers flexibility and is a great opportunity for a dedicated and compassionate clinician to make a meaningful difference in the lives of residents while working in a supportive, therapist-led environment.

Your Responsibilities

  • Deliver compassionate and effective occupational therapy treatments under OT supervision

  • Implement individualized treatment plans to improve functional independence and quality of life

  • Support residents in achieving personal goals and progress milestones

  • Accurately document care and maintain compliance standards (experience with Net Health a plus)

  • Collaborate with the therapy and nursing teams to ensure patient-centered outcomes

  • Educate residents and families on safety and daily living strategies

  • Participate in team meetings and contribute to care planning

What You Bring

  • Current Wisconsin COTA license

  • Strong clinical skills and a genuine passion for geriatric care

  • Excellent communication and teamwork abilities
