Chase Memorial is seeking dedicated and compassionate Occupational Therapist's (OT) to join our team on a part-time and per diem basis. The OT will evaluate and treat patients with a variety of physical, developmental, or cognitive conditions, helping them improve their ability to perform daily activities and enhance their overall quality of life.

Job Summary:

Part-time- 20 hours per week

Per Diem positions available

Key Responsibilities:

  • Conduct initial evaluations and develop individualized treatment plans

  • Provide therapeutic interventions to improve activities of daily living (ADLs), fine motor skills, coordination, and cognitive functioning

  • Educate patients and caregivers on treatment techniques and home exercise programs

  • Monitor patient progress and adjust treatment plans as needed

  • Maintain accurate and timely documentation in compliance with regulatory and organizational standards

  • Collaborate with interdisciplinary team members to ensure optimal patient outcomes

  • Ensure a safe, clean, and supportive treatment environment

Qualifications:

  • Master’s or Doctorate degree in Occupational Therapy from an accredited program

  • Current state licensure as an Occupational Therapist

  • CPR certification (or willingness to obtain)

  • Strong communication and interpersonal skills

  • Ability to work independently and adapt to varying patient needs

  • Experience in a Long-Term Care or Skilled nursing preferred but not required
