Home Health Occupational Therapist OT
Job Summary:
PathWell Health, a trusted and community-focused home health care provider, seeks a compassionate and experienced Home Health Occupational Therapist OT to join our team in San Diego, California. Responsibilities:
Develop, implement, and evaluate plans of care in partnership with clients, representatives, caregivers, and physicians
Provide direct occupational therapy services in accordance with established plans of care
Educate and instruct clients, family members, or other representatives on rehabilitation care and activities to promote health, safety, and independent living
Accurately document observations, interventions, and evaluations pertaining to client care management and services provided
Select activities that promote work and life-management skills within clients' mental or physical capabilities
Recommend changes to patients' work or living environments to ensure their needs and capabilities are met
Engage in research activities and participate in formal educational training related to therapy
Minimum Qualifications:
Current and valid California Occupational Therapist license
Recent work experience in home health (preferred)
Driver's License, Car Insurance, Car Registration
Strong clinical assessment, organizational, and communication skills
Ability to handle multiple assignments simultaneously
Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing, and documentation
Bilingual (Spanish) preferred
Excellent oral and written communication skills
Excellent customer relationship skills

Compensation:
Salary: $90,000 - $120,000 annually
