Centennial Healthcare Group is seeking an experienced and highly motivated Regional Nurse Consultant (RN) to support our skilled nursing facilities in Colorado, primarily in the Alamosa, CO area. What You’ll Do:
- Conduct regular on-site visits to assess clinical operations and ensure regulatory compliance
- Collaborate with facility leadership to evaluate nursing systems and recommend improvements
- Train and mentor Directors of Nursing and nursing staff on best practices, procedures, and compliance
- Assist with state survey prep, corrective action planning, and post-survey follow-up
- Participate in screening and onboarding for nurse leadership positions
- Serve as a trusted advisor to ensure residents receive the highest quality care
- Perform audits, identify gaps, and implement performance improvement strategies
- Work closely with the VP of Clinical Services and fellow Regional Nurse Consultants on company-wide clinical initiatives
What We’re Looking For:
- Active RN license in Colorado or compact state license (required)
- 3–4 years of experience in a skilled nursing or long-term care environment
- Multi-facility or regional consulting experience preferred
- Strong working knowledge of state and federal long-term care regulations
- Excellent communication, training, and problem-solving skills
- Comfortable traveling between facilities in and around the Alamosa, CO region
- High level of initiative, accountability, and professionalism
