What You'll Do

As an ADvantage Nurse LPN, you support patient independence and safety through high‑quality clinical care. Working under the supervision of an RN, you will:

  • Complete initial assessments, nursing evaluations, admission paperwork, and task sheets
  • Educate patients and families on services, policies, fall prevention, and care needs
  • Coordinate with members and case managers to ensure appropriate care plans
  • Provide authorized clinical services such as medication preparation, skin monitoring, and nail care for qualifying members
  • Maintain strict infection control practices
  • Document all care in accordance with organizational standards
  • Reassess patient needs regularly and adjust care plans as needed
  • Ensure compliance with all regulations and uphold confidentiality at all times

Qualifications

  • Active LPN license with at least six months of monitoring‑visit experience
  • CPR certification within 60 days of hire
  • Valid driver's license, clean driving record, reliable transportation, and proof of insurance
  • Ability to travel long distances and adapt to frequent schedule changes
  • Need to be able lift, push, pull and carry up to 50lbs.
  • Need to be able lift, push, pull and carry up to 20lbs 70% of the time and 30lbs up to 25% of the time.
  • Preferred experience: home health, critical care, or emergency room

Schedule

Full‑time, Monday-Friday, 8:00 a.m.-5:00 p.m.
