Bluestem PACE is looking for a full-time Physical Therapist to join our team! This position will support both our McPherson and Hutchinson locations and follows a Monday - Friday schedule from 8:00am - 4:30pm.

Position Overview:



The Physical Therapist will be responsible for evaluating participants and administering tailored physical therapy treatments while monitoring their progress. Additionally, the Physical Therapist will conduct assessments, develop care strategies, and coordinating care delivery while maintaining communication with the team.



 



Essential Job Functions:




  • Evaluates participants and administers physical therapy treatments.

  • Records progress and instructs participants and/or family in care.

  • Supervises work activity of restorative aides, assistants, and students.

  • Will work in the PACE Center and in participant homes.

  • Conducts initial assessment and period reassessments of participants.

  • Develops plan of care for participants.

  • Coordination of 24-hour care delivery.

  • Regularly informs the interdisciplinary team (IDT) of the medical, functional, and psychosocial condition of each participant.

  • Remains alert to pertinent input from other team members, participants’ caregivers.

  • Documents change in participant’s medical record consistent with documentation policies.

  • Evaluates participant needs for durable medical equipment (DME), prescribes such equipment, and monitors its appropriate use and ongoing condition to support participant mobility and rehabilitation goals.
