Cascade Community Healthcare (CCH) has been providing treatment services to Lewis County and surrounding areas for more than 50 years. Cascade currently has six treatment locations in Lewis County.
We are looking for an SUDP to join our team! The Substance Use Disorder Professional is responsible for managing the care of adult individuals who are in crisis and stabilizing from crisis situations. The role is responsible for providing substance use disorder assessments, diagnosis and treatment planning. This position collaborates closely with an interdisciplinary team and treatment team to coordinate detox services.
SHIFTS AVAILABLE:
Day Shift, Thursday - Saturday: 3 - 12s, 10:00am - 10:30pm
NOC Shift, Thursday - Saturday: 4 - 12s, 10:00pm - 10:30am
NOC Shift, Sunday - Wednesday: 4 - 12s, 10:00pm - 10:30am
Based on current utilization levels within the Crisis Relief Center, staffing schedules utilizing 3x12 and 4x12 shifts have been implemented to support operational needs. Scheduling and total weekly hours remain subject to change based on ongoing utilization, patient care demands, and organizational needs.
PAY SCALE:
$30.14 - $36.74, DOE.
WHAT YOU NEED TO GET THE JOB DONE:
- Bachelor’s degree in behavioral health, social work, or related discipline required.
- Current certification as a Substance Use Disorder Professional.
- Must have working knowledge of ASAM Patient Placement Criteria.
- Must be able to pass WA State Patrol background check. A national background check may be required if have not lived in WA state consecutively the last three years.
- Must demonstrate the ability to work as part of a team.
- Must be able to represent the agency in a positive and professional manner.
- Must be able to communicate clearly, both verbally and in writing.
- Must comply with agency policies and procedures
- Must be able to pass Violence Prevention Training.
- Must maintain current phone number and mailing address with Human Resources.
WHAT YOU'LL DO:
- Conducts Substance use Disorder Assessments in accordance with ASAM, or other assessment tools, and organizational policies and procedures.
- Educates and informs individuals of consents for treatment and authorizations for use and disclosures.
- Participates regularly in interdisciplinary treatment team meetings.
- Collaborates with the treatment team to develop and implement an individualized person-centered treatment plan that is appropriate for brief intervention. Complete the signed treatment plan and review with each individual and any other involved parties.
- Gathers collateral from the individual and involved parties to provide a thorough assessment of what an individuals' needs are for treatment and discharge.
- Actively coordinates care and provide referrals for individuals to ensure services are coordinated with both facility staff and external providers.
- Identify and address gaps in service needs for participating parties in service delivery to the individual and make appropriate recommendations to meet the individuals’ needs upon discharge.
- Collaborates with interdisciplinary treatment team to engage, monitor and communicate with the individual ensuring that safety is maintained while meeting the individuals' needs.
- Conducts brief intervention, individual, and family counseling within scope of practice.
- Participates in redirecting individual behavior while maintaining a safe milieu when necessary.
- Maintains the Electronic Health Record (EHR), including documentation of activities performed as part of the service delivery process.
- Obtain signatures from individuals regarding treatment, as necessary.
- Ensures documentation complies with best clinical practices, contracts, regulatory, and other legal requirements.
- Maintains a working knowledge of the Washington State behavioral health system and potential funding sources.
- Participates in individual and group supervision as required.
- Participates in ongoing education, including in-services, training, and other activities to maintain and improve competency.
- Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor.
- Performs all other duties as assigned.
PHYSICAL DEMANDS:
- Primarily sedentary, sitting at workspace.
- Physical interventions in response to aggressive and self-injurious behavior.
- Successful completion of MOAB training (Management of Aggressive Behavior).
- Including but not limited to pivoting, knee squats, shuffling front and backwards, assisting a person to the floor, stabilizing people in standing, seated, lying and supine positions.
- While performing the duties of this job, the employee is regularly required to see, talk, and hear.
