Job Description

 Unit Coordinator - Full-Time 

8am - 4:30pm | Sub-Acute Rehab (Retirement Home) | Medford, NJ 

$17.23/hr to $19.81/hr

 

About the Role:

The Unit Coordinator, working under the supervision of the Resident Care Manager and the Director of Nursing, is responsible for ensuring the seamless coordination of resident health services. They facilitate communication between residents and staff, administer resident services, manage medical records, process payroll, and provide essential follow-up and reporting. This role plays a vital part in the comprehensive care approach within the facility.

 

Scheduling: 

  • Full-Time: 8am - 4:30pm; 37.50 hrs per week

Rate:

  • $17.23/hr to $19.81/hr to Start (depending on experience) and pay advancement potential depending on annual performance. 

 

Responsibilities:

  • Coordinates and schedules Resident appointments including but not limited to; paperwork preparation, transportation arrangement, companion service arrangement, and follow up care coordination of services.
  • Prepares admission chart with proper paperwork.  May include obtaining past history and physicals from previous charts.  Enters demographic information into computer software
  • Assist with department orientation of new staff
  • Maintains medical and office supply par levels for unit.  Orders supplies as needed and checks for expiration of supplies on a weekly basis.
  • Breaks down discharged resident’s charts and forwards them to the Medical Records Coordinator.  Maintains, updates, reviews and thins charts in accordance with state and federal regulations. 
  • Reports medical records/chart issues to the Medical Records Coordinator
  • On a timely basis, submits to the Director of Nursing employee data for payroll processing which has been accurately prepared with appropriate notation related to the payment of overtime, leave, shift differentials, or other items, as directed; accurately enters schedules and scheduled corrections into payroll computer software
  • Collaborates with RNAC/MDS Coordinator in planning resident MDS and care conference schedule.

 

Education/Experience Requirements:

  • High School Graduate
  • Computer literate with competency in Word and Excel 
  • 1-3 years of administrative experience, preferably in a healthcare setting.
  • Current Certified Nursing Assistant for the State of NJ preferred. 
  • Must possess excellent customer service, time management, organizational, planning and multitasking skills
  • Must be able to hear, read, write and speak English

 

Physical Requirements and Environmental Conditions:

  • Must be able to lift up to 25 lbs. on a regular basis
  • Sits, walks, bends, lifts, stoops and stands the majority of the day
  • Moves intermittently during working hours
  • Duties involve manual dexterity for up to 85% of the time, must be able to sit and use a computer, telephone, and other office equipment 

 

License and Regulatory Requirements:

  • Must obtain all required annual industry related / regulatory immunizations 
  • Must complete mandatory training as required
