Job Summary



The Facility Based Hospice Services Certified Nursing Assistant (CNA) provides high‑quality hospice care to patients residing in long‑term care facilities, assisted living communities, and skilled nursing facilities. The CNA functions as an integral member of the interdisciplinary hospice team and delivers personal care services that support comfort, dignity, and quality of life for patients with life‑limiting illnesses.  This position requires professionalism, clinical competence, and the ability to collaborate effectively with both Delaware Hospice staff and facility personnel.



 



Responsibilities what you do –



Patient Care




  • Provide personal care services including bathing, grooming, oral hygiene, dressing, toileting, and repositioning in accordance with the established plan of care.

  • Assist with mobility, transfers, and safe ambulation as appropriate.

  • Ability to bend, squat, driving for extended periods of time, lift and turn patients, able to lift a minimum of 50 lbs

  • Observe and report changes in patient condition, behavior, or needs to the assigned hospice Registered Nurse (RN) and facility staff.

  • Support comfort-focused interventions such as skin care, pressure injury prevention, and non‑pharmacologic symptom relief measures.

  • Maintain a clean, safe, and therapeutic environment for patients.



Interdisciplinary Collaboration




  • Coordinate care with facility staff to ensure continuity and adherence to the hospice plan of care.

  • Communicate promptly and professionally with the hospice RN Case Manager regarding patient status and care needs.

  • Participate in interdisciplinary team meetings and case discussions as requested.

  • Foster positive working relationships with facility partners, patients, and families.



Documentation and Compliance




  • Accurately document all care provided in accordance with Delaware Hospice policies, regulatory requirements, and facility protocols.

  • Adhere to all infection control, safety, and confidentiality standards.

  • Maintain compliance with state and federal regulations governing hospice and CNA practice.



 



Professional Conduct




  • Demonstrate respect for patient rights, cultural values, and end‑of‑life preferences.

  • Uphold the mission, values, and ethical standards of Delaware Hospice.

  • Maintain professional boundaries and conduct at all times.
