As the Wellness Advocate, you will play a pivotal role in individuals' lives, making a significant impact through:

  • Conduct outreach and engagement activities in greater capital region based on member lists provided by CDPHP
  • Assist members with linkage and referral to necessary preventive, primary, screening, and other care resources
  • Represent the agency in keeping with agency mission and standards of professionalism and ethics
  • Provide information about diseases, medications, psychosocial issues and harm reduction education to patients/members
  • Use health information technology to link to services and resources and communicate among team members, providers, and patients/members and their families/caregivers
  • Manually and/or electronically document activities and patient/member information and interventions as directed

Wellness Advocate Minimum Salary: $聽19.49

Wellness Advocate Maximum Salary: $聽19.49

Wellness Advocate Schedule: Mon-Fri 8:30AM - 4:30PM

Qualifications:

  • High School Diploma or GED
  • 1 - 3 years of experience working with a diverse population
  • Must have a personal cell phone, valid driver鈥檚 license, proof of current car insurance and registration and access to a reliable, working vehicle
  • Bilingual in Spanish and English is highly desirable
