Job Description

GENERAL SUMMARY:

Under the direction of the Executive Director, the Social Worker aids residents and their families with personal and environmental difficulties which predispose illness or interfere with obtaining the highest level of physical, emotional and psychosocial function.

The Social Worker must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the residents served by his/her department – adult through geriatric.  The Social Worker must demonstrate knowledge of the principles of growth and development over the life span; possess the ability to assess data reflective of the resident's status; interpret the appropriate information needed to identify each resident's requirements relative to their age-specific needs; and to provide the care needed as described in the Social Service Department's policies and procedures.

EDUCATION:

Bachelor’s degree in Social Work or related field required.

Master’s degree in Social Work or related field preferred.

WORK EXPERIENCE:

Minimum of two (2) years of experience in a Social Work Department health care setting required.

Skilled Nursing experience preferred.

SPECIAL EMPLOYMENT REQUIREMENTS:

PA license to practice Social Work is preferred.

Valid PA Driver’s License required for travel within community.

Must be cleared by PA State Police criminal history check.

Good verbal and written communication skills and interpersonal skills are required for effective communication with residents, families, staff, physicians, and outside agencies.

Must have strong understanding of family dynamics and individuals as they relate to illness and disability.

Must demonstrate an understanding of the medical and emotional problems of the physically disabled through the use of Human Relations and proven theories of Social Work practice.

Must demonstrate an understanding and availability of community resources to facilitate their residence and then in coordinating discharge from an inpatient to outpatient setting while maintaining a safe environment for the resident.

WORKING CONDITIONS:

Works in well-lit office environment and other hospital departments (Nursing, OT, PT, etc.).
