Registered Nurse (RN) Start of Care for Home Health

Job Summary:

Deer Meadows Home Health is seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Start of Care Nurse for Home Health. As a vital member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in their homes. Your expertise will enable us to deliver exceptional outcomes and improve the overall health and well-being of our patients.

Registered Nurse (RN) Start of Care Responsibilities:

  • Conduct initial assessments and develop comprehensive care plans for patients in their homes

  • Implement and monitor treatment plans, ensuring continuity of care and communication with patients, families, and healthcare providers

  • Collaborate with physicians, therapists, and other healthcare professionals to develop and update patient care plans

  • Educate patients, families, and caregivers on disease management, medication regimens, and rehabilitation strategies

  • Identify and address patient safety concerns, adhering to evidence-based practices and guidelines

  • Document patient progress and care activities accurately and timely, according to agency policies and regulations

  • Maintain a positive and respectful attitude, promoting a supportive and inclusive environment for patients, families, and colleagues

Registered Nurse (RN) Start of Care Requirements:

  • Current licensure as a Registered Nurse in the state of Pennsylvania

  • Prior experience required; experience in home health or a similar setting desired

  • Strong communication, interpersonal, and problem-solving skills

  • Ability to work independently and as part of a team

  • Willingness to learn and adapt to new technologies and procedures
