Job Description

PURPOSE: The purpose of this position is to provide general nursing care and leadership, working collaboratively with physicians and multidisciplinary team members. The RN?will promote and restore patient?s health as they assess, provide, delegate, evaluate and coordinate comprehensive professional nursing care through the use of the nursing process for patients of all ages, gender, cultures, and background; provide physical and psycho-social support to patients, friends, and families. As well as oversee clinical care of other personnel including LPN?s, care co-coordinators and MA?s.



Salary : $ 42.00 (base hourly Rate)



ESSENTIAL DUTIES AND RESPONSIBILITIES:




  • The RN is responsible for providing safe, compassionate, and comprehensive nursing care to patients and their families with complex healthcare needs. Duties and responsibilities include the following. Other duties may be assigned.

  • Follows all PCMH procedure and policies in a culturally sensitive manner.

  • Able to cover all duties as a registered nurse.

  • Assists the Medical Provider and interprets medical recommendations and reinforces Provider?s instructions.

  • Provides daily over-site of support staff, including but not limited to:


    • Developing employee schedules to ensure adequate clinic and lunch coverage, inventory assessment etc.




    • Oversees the performance of clinical competencies.




    • Provides daily delegation tasks and provides oversight to ensure medical assistants adhere to ACH?s performance expectations.






  • Takes medical and social histories and vital signs, maintains patient flow to facilitate provider and lessen wait times.




  • Instructs patients regarding laboratory and diagnostic procedures and assists with laboratory tests.




  • Explains procedures and treatments to patients to establish nurse-patient rapport.




  • Creates an understanding of the nurse navigating patient care; educates, empowers, and encourages compliance with appointments, laboratory testing and diagnostic imaging.




  • Provides emotional support continuously as needed to solidify patient-nurse trusting/working relationship and to decrease anxiety experienced by patients.




  • Reviews normal labs and diagnostic results with patients to increase knowledge base and empower them in the best way possible.




  • Triages all walk-in patients/ problem telephone calls by conducting and documenting acuity of illness/medical problem and taking appropriate steps to a successful/safe resolution.




  • Instructs and educates patients regarding laboratory diagnostic procedures and assists with laboratory test(s) performed on site.




  • Responsible for maintaining adequate departmental inventory of vaccinations and supplies; Maintain VFC database.




  • Ensures departmental adherence to Infection Control practices and compliance with laboratory procedures





Requirements



Computer Skills - Must be computer savvy; having a general knowledge of Microsoft Office. The Registered Nurse will be required to undergo ECW training.

Education and/or Experience: Bachelor of Science in Nursing (BSN) preferred, or Associate Degree in Nursing (ADN) required.? Active NJ RN License,?BLS/ACLS certification.?Prior clinical experience as a Registered Nurse is preferred, especially in?an out-patient setting.

Physical Demands: While performing the duties of this Job, the employee is regularly required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to sit; climb or balance and stoop, kneel, crouch, or crawl. The employee must regularly lift and /or move no more than 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
