What You’ll Do
  • Coordinate ongoing assessments of patient clinical status and level of care
  • Evaluate and adjust care plans to ensure realistic, outcome-driven goals
  • Collaborate with patients, families, interdisciplinary teams, and payors
  • Manage resource utilization including services, consults, and length of stay
  • Coordinates all aspects of discharge planning process and transitions of care
  • Track and analyze outcomes data to improve care delivery
  • Serve as a resource for staff on managed care and regulatory guidelines
  • Maintain accurate, timely, and compliant documentation
What We’re Looking For
  • Active Massachusetts LPN, RN or Social Work license required
  • 1+ year experience in two or more of the following:
    • Case management
    • Skilled nursing or sub-acute care
    • Discharge planning
    • Managed care / Medicare
  • Strong understanding of managed care and reimbursement
  • Excellent communication and critical thinking skills
  • Ability to work independently and collaboratively
