Position Summary

The Wound Care Nurse is responsible for the assessment, treatment, and ongoing management of residents with complex wounds, pressure injuries, and other skin integrity concerns. This position ensures the highest level of wound care in compliance with clinical standards, physician orders, and state/federal regulations, while promoting residents� comfort, healing, and overall quality of life.

Essential Duties and Responsibilities

  • Perform comprehensive assessments of wounds, skin conditions, and risk factors using evidence-based practices.
  • Develop, implement, and regularly update individualized wound care plans in collaboration with physicians and interdisciplinary team members.
  • Providing direct care includes dressing changes, treatments, and measurements of wound progress.
  • Monitor and document residents� wound healing, status changes, and interventions in the electronic medical record.
  • Educate residents, families, and staff on wound prevention, infection control, and skin integrity maintenance.
  • Coordinate referrals to physicians, specialists, or outside providers as needed.
  • Participate in skin rounds, audits, and care plan meetings to ensure continuity of care.
  • Maintain accurate, timely documentation in compliance with facility policies, state, and federal guidelines.
  • Serve as a resource and mentor to nursing staff on best practices for wound and skin care.
  • Support compliance with infection prevention and quality assurance initiatives related to wound management.

Qualifications

  • Current, unencumbered Licensed Practical Nurse (LPN) license in the state of Tennessee.
  • Wound care certification (e.g., WCC, WOCN, or equivalent) is preferred.
  • Minimum of 1�2 years of clinical nursing experience, preferably in long-term care or wound management.
  • Strong assessment, clinical reasoning, and organizational skills.
  • Excellent communication and documentation abilities.
  • Commitment to resident-centered care and teamwork.

Physical Requirements

  • Ability to lift, move, and position residents up to 50 pounds with assistance.
  • Frequent standing, walking, bending, and reaching during shift hours.
  • Visual acuity and manual dexterity are required for patient care and documentation tasks.
