Position Summary:
The Bilingual Community Health Worker (CHW) plays a critical role in advancing equitable access to mental and behavioral health services for uninsured and underinsured adults across rural and underserved communities. As part of the MindCare Now initiative, the CHW will engage clients across CFHI programs, build trust, reduce barriers to care, and support enrollment and ongoing participation in a virtual behavioral health platform (Talkspace).
This position is essential to improving access, engagement, and continuity of care by addressing social determinants of health, supporting digital access, and ensuring culturally and linguistically appropriate outreach and navigation.
Principal Responsibilities:
Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with co-workers
Conduct outreach and engagement across CFHI programs (WIC, Early Intervention, Nurse Family Partnership, Monmouth ACTS Resource Connect, HIV Case Management, Health Families, Parents as Teachers)
Build trusting relationships with clients through culturally responsive, trauma-informed approaches.
Provide education on mental and behavioral health services, reducing stigma and increasing awareness
Support program marketing efforts in collaboration with internal teams
Assist clients with enrollment and account set up
Work closely with the case manager to coordinate client care and service delivery
Collaborate with Talkspace team members to ensure smooth client onboarding and engagement
Job Competencies & Minimum Qualifications:
1. High School diploma/GED equivalency required
2. Minimum one year of community involvement and/or working with clients, or an equivalent combination of education and experience
3. Knowledge of cultural and lifestyle diversity issues with ability to network with a specific population
4. Bilingual; Spanish preferred
5. Excellent verbal and written communication skills
6. Ability to work both independently and as part of a team; Strong organizational and interpersonal skills
7. Valid N.J. driver’s license and reliable transportation

Working Conditions/Physical Demand:
Works indoors/outdoors with client/customer contact and with use of business office machines such as: telephone, computer, printer, copier, fax machine, email, and voicemail
In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $30,000.00 to USD $65,000.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
