ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Directly oversees all aspects of admission process for all market skilled nursing centers and market admissions associates.
  • Develops and implements training programs and professional development plans to educate Admissions Coordinators, Admissions Assistants and other team members on skilled nursing admission guidelines and processes.
  • Participate in Nursing Center QAPI process. 
  • Responsible for meeting established goals related to admissions for market skilled nursing centers, referral source relationship development, outreach program implementation and other quality metrics established.
  • Communicates with organization personnel, especially designated market Patient Transition Directors regarding patient transitions.
  • Develops and coordinates admissions associates in strategic outreach activity in market in order to facilitate referrals to skilled nursing centers. 
  • Performs strategic outreach events in market in order to increase community awareness and facilitate referrals to skilled nursing centers.
  • Reviews monthly analytic data.  Tracks information to determine market trends, and develops action plans based on data relative to key performance indicators.
  • Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.
  • Attend and participate in mandatory in-services.
  • Honor patient’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.
  • Comply with Corporate Compliance Program.
  • Report job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.
  • Follow established safety regulations, to include fire protection/prevention, smoking regulations, infection control, etc.
