Job Description

Stress Care of New Jersey is an outpatient mental health and substance Abuse treatment center licensed with the NJ Dept of Human Services and Joint Commission accredited for individual and group work to help clients primarily in from Monmouth county.

As a Certified Peer Recovery Specialist you will provide community assistance through the implementation of the individualized care plans for each client and/or family enrolled in Stress Care's Behavioral Health Home services.

Responsibilities include

  • Ensuring engagement and retention by means of client outreach
  • Completing initial screening and orientation for new clients
  • Performing risk assessment and safety planning
  • Assisting with application of services, arrange transportation, and escort to appointments as necessary
  • Liaising with medical professionals and others, on behalf for the clients as needed
  • Maintaining communication with client, family, and community providers to problem solve barriers
  • Submitting Quality Assurance data in accordance with CMS required quality measures
  • Participating in weekly supervision and daily staff meetings

Requirements

  • Active Peer Recovery Specialist Certification [REQUIRED]
  • High School diploma/GED equivalent [REQUIRED]
  • 1 year case management experience [PREFERRED]
