Compensation: Pay per visit, $44 - $50 per routine visit, processed bi-weekly.
Shift Type: Full-time, Monday - Friday, 8 AM to 4 PM (flexible).

Summary:
We are looking for a compassionate Certified Occupational Therapist (COTA) to work one-on-one with our patients in their home environment to give proper education and therapy to allow our patients to become more independent. The COTA for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.
Responsibilities:
Administer occupational therapy treatments that help patients reach their care goals in accordance with the Plan of Care and physician orders.
Assist in the implementation of programs and activities established by the OT to restore, reinforce, and enhance task performances and promote health and self-sufficiency.
Design and/or adapt equipment to the patient’s working and/or living environment.
Evaluate effects of occupational therapy treatments; recommend adjustments and modifications in consultation with the OT and physician.
Participate in and contribute to case conferences and care coordination/discharge planning. Act as a resource to other healthcare personnel in order to meet patient’s needs.
Document patient care services by charting in patient and department records.
Qualifications:
1-year experience as a COTA in home care, acute care, or rehab
Current state license as a COTA.
Proof of current CPR certification and valid Driver’s license.
Ability to work and flexible schedule.
Excellent interpersonal skills.
