Health Concepts and Nursing Placement are seeking a dedicated Rhode Island Registered Dietician to provide one-on-one care to patients in their homes.
Duties and Responsibilities:
- Counsels and assists nursing staff, clients or caregivers on dietary practices and requirements needed for optimal nutritional status.
- Develops a nutritional plan based on patient’s evaluations and or goals recommended by the physician and agreed upon by the patient or caregiver.
- Attends interdisciplinary team conferences as appropriate.
- Attends, participates in or conducts staff development education programs/in-services.
- Accurately documents observations, nutritional assessment, dietary plan of care and goals as it relates to patients on day services are rendered. Submits documentation on a weekly basis. Case conferences with primary nurse immediately after the consultation.
- Performs other services appropriate for the discipline as requested by Administration.
- Complies with Contract Agreement and Agency policies and procedures.
- Complies with HIPPA Policy.
- Assess the effects of end of life disease on hydration/nutrition.
Qualifications and Experience:
- B.S. degree in nutrition and/or dietetics from a program approved by the American Dietetic Association.
- A minimum of two years clinical experience in nutrition and dietetics.
- Currently licensed as a registered Dietitian/Nutritionist in the State of Rhode Island.
- Knowledge of therapeutic diets.
- Keeps current on the latest changes in therapeutic diets.
- Knowledge of management of illness through therapeutic diets.
