Job Description: Registered Nurse (RN)

Position Overview:
All Embracing Home Care is seeking a compassionate and dependable Registered Nurse (RN) to provide high-quality, patient-centered care in a home setting. This role focuses on promoting patient independence, improving health outcomes, and delivering personalized care tailored to each individual’s needs. The RN will also play a key role in supporting and training Care Managers through medication practicum and clinical guidance.

Key Responsibilities:

  • Perform initial and ongoing comprehensive patient assessments
  • Develop, implement, and revise individualized care plans
  • Provide skilled nursing care, including medication administration and treatments
  • Monitor patient progress and document clinical findings accurately
  • Identify changes in patient condition and communicate with physicians and care teams
  • Educate patients and family members on disease management, medications, and care techniques
  • Conduct medication practicum and competency training for Care Managers, ensuring safe medication administration practices
  • Provide clinical oversight, guidance, and support to Care Managers as needed
  • Ensure compliance with all healthcare regulations and agency policies
  • Coordinate care with other healthcare professionals and support staff

Qualifications:

  • Active Registered Nurse (RN) license in ND
  • Previous nursing experience required; home care or community health experience preferred
  • Experience in training or mentoring staff is highly desirable
  • Strong clinical judgment and critical thinking skills
  • Excellent communication, teaching, and patient education abilities
  • Ability to work independently and manage a flexible schedule
  • Valid driver’s license and reliable transportation

Work Environment:

  • Provides care in patients’ homes within assigned service area
  • Requires travel between patient locations

Shift would consist of 2-3 (12 hour) shifts a week with the ability to pick up additional hours.
