This position is responsible for acting as a liaison between primary care physicians and specialists to streamline the referral process. This includes, but is not limited to, managing and coordinating patient referrals within a healthcare facility, ensuring patients are seamlessly transferred to specialists or other healthcare providers by reviewing medical records, identifying appropriate specialists based on patient needs and insurance coverage, scheduling appointments, obtaining necessary authorizations, and communicating with both patients and providers to facilitate timely access to care. The person in this role work in two offices within a five mile radius and must be comfortable working independently.
Job Summary
1. Greets patients professionally both in person and on the phone.
2. Reviews patient medical records to determine the need for specialist consultations or diagnostic tests.
3. Identifies appropriate specialists to match patients based on their condition and insurance plan.
4. Receives and reviews incoming referral requests from physicians, specialists, and other healthcare providers.
5. Obtains prior authorizations by contacting insurance companies to verify coverage and obtain necessary pre-certifications for referrals.
6. Coordinates appointment times and locations with specialists for patients.
7. Manages patient communication by informing patients about their referral appointments, including details like date, time, location, and any pre-visit requirements.
8. Maintains accurate records of patient referrals, including referral forms and communication logs.
9. Follows up with patients to ensure they attend scheduled specialist appointments and addresses any concerns they may have.
10. Works closely with primary care physicians, specialists, and other healthcare staff to facilitate smooth referral process.
11. Provides excellent customer service by answering patient and provider inquiries promptly and professionally, with an emphasis on creating a welcoming and patient-centered environment.
12. Assists patients in understanding the referral process, ensuring they feel informed and supported at every stage.
13. Addresses and resolves patient concerns related to referral appointments, scheduling conflicts, and insurance issues in a timely and empathetic manner.
14. Tracks and reports on referral outcomes, including appointment completion and patient feedback, as required by management.
15. Maintains up-to-date knowledge of insurance requirements, referral guidelines, and the practice's policies.
16. Performs any other duties as assigned.
