Guided by our values of integrity, respect, and excellence, we are seeking an experienced Community Education Liaison to expand our reach and foster meaningful partnerships with healthcare providers and community organizations.
The ideal candidate will be a dynamic and compassionate professional with a strong background in healthcare outreach, relationship-building, and community engagement. They will have the ability to cultivate strategic partnerships with physicians, case managers, and community organizations, while effectively representing Big Bend Hospice at events and networking opportunities. A successful Community Education Liaison will demonstrate excellent communication skills, a deep understanding of hospice care, and the ability to use data and CRM tools to inform strategy and support growth. Most importantly, they will embody Big Bend Hospice’s values of integrity, respect, and excellence in every interaction.
Position Duties and Responsibilities:
As a Community Education Liaison, you will:
- Build and nurture relationships with physicians, case managers, and healthcare providers to drive qualified referrals and admissions.
- Going to Hospitals and doing informational with families, patients, and signing consents when needed and getting verbal CTI.
- Represent Big Bend Hospice at community events, health fairs, and professional networking opportunities to enhance brand awareness.
- Collaborate with the Director of Access and Growth to analyze market trends, develop business plans, and optimize territory coverage.
- Use CRM tools to track interactions, report progress, and inform strategic decisions.
- Act as a liaison between patients, families, and providers, assessing eligibility, explaining hospice care, and coordinating admissions.
- Provide clinical education and in-services to healthcare providers in collaboration with the business development team.
- Support BBH’s mission by maintaining accurate documentation and participating in team meetings and quality improvement initiatives
Work Schedule:
- Monday-Friday; 8:00 am- 5:00 pm
Travel Requirement: You must have a reliable vehicle and be willing to be dispatched throughout our service areas.
This position will cover the following counties below and will require travel to various community locations. These will include but are not limited to the following: physician offices, skilled nursing facilities, assisted living facilities, specialty doctor offices, senior centers

- Jackson
- Gulf
- Holmes
- Washington
- other counties as assigned
