ROLE

Using the principles of person-centered care, the registered nurse is responsible for providing services to referred stroke clients in a variety of settings, with a focus on symptom management and stroke prevention. The nurse provides client-specific consultation and education, training and in-services for service providers, caregivers and families.

REQUIRED EDUCATION & CERTIFICATION(S)

  • Registered member in good standing of the College of Nurses of Ontario
  • Current Basic Life Support Certification
  • Valid Ontario Driver’s License

REQUIRED SKILLS & EXPERIENCE(S)

  • Clinical expertise and experience in stroke rehabilitation and demonstrated knowledge of current stroke rehabilitation best practices
  • Knowledge of community service providers within Huron and Perth counties
  • Understanding of Canadian Stroke Best Practices
  • Demonstrated knowledge of current evidence and effective strategies for vascular risk factor monitoring and management and secondary stroke prevention
  • Working knowledge of and experience with theories and principles guiding adult learning and patient self-management approaches
  • Knowledge of the principles of adult education
  • Sound understanding of the role of a variety of rehabilitation professionals in the provision of stroke rehabilitation
  • Excellent assessment skills and development of personalized patient informed treatment plans
  • Demonstrated ability to prioritize and function both independently and collaboratively with the inter-professional care team
  • Knowledge and skill caring for stroke patients and individuals with complex medical needs
  • Demonstrated commitment to ongoing professional development
  • Well-developed interpersonal, communication and teaching skills
  • Ability to plan, think critically, organize, and problem solve
  • Strong interpersonal skills with a demonstrated ability to establish positive relationships with clients, families, caregivers and community services
  • Excellent English verbal and written communication skills using different mediums, such as telephone, in-person and video
  • Evidence of continuing education and professional development
  • Proficiency with computer applications including Microsoft applications and working knowledge of or the ability to quickly comprehend the Meditech Information System

DESIRED SKILLS, EXPERIENCE(S) & EDUCATION

  • Completion of Canadian Hemispheres course preferred
  • Experience in both hospital and community service delivery is an asset
