Job Description

Starting Rate: $27.25 - $31.75 - Based on Experience  Shift Differential and Weekend Differential available!!!!

Sign-On BONUS - Earn up to $5,000

 

DESCRIPTION:

  • Manages a designated group of nursing employees on a nursing unit and delegates/supervises resident care. 
  • Administers medications/treatments on the assigned unit

DUTIES AND RESPONSIBILITIES: 

  • Works closely with the Nurse Manager/Nursing Supervisor in all aspects of resident care to ensure continuity of resident care.
  • Administer medications and treatments in accordance with physician orders and facility protocols
  • Monitor and document residents' health status, reporting changes to the RN or attending physician promptly
  • Perform skilled nursing tasks including wound care, catheter care, and tube feedings
  • Communicate clearly with residents, families, and interdisciplinary team members
  • Maintain accurate and timely documentation in the electronic health record (EHR)
  • Follow all state, federal, and facility regulations, including infection control and HIPAA standards
  • Supervises CNA's
  • Checks medication/treatment supplies and reorders as necessary.
  • Assists in emergency care as necessary.
  • Assists the Nurse Manager/Nursing Supervisor with evaluation of resident's needs, condition and care.
  • Relieves CNA's for breaks and mealtime when necessary.
  • Monitors and assists resident's during meal times
  • Follows proper policy and procedure with maintenance of controlled substances (i.e.: administration, count, documentation of effectiveness).
  • Assists the CNA's with resident care as needed.
  • Other related duties that may have become necessary or as directed by the supervisor, department head and/or administration.

Qualifications:             

  • Current L.P.N. licensed in the State of New York
  • Current CPR/BLS certification
  • Strong clinical assessment and communication skills
  • Ability to work collaboratively in a fast-paced, team-oriented environment
  • Experience in a long-term care or skilled nursing setting preferred
  • CPR certification required
