Job Description

Purpose: The Hospice LPN is a member of the Interdisciplinary Team (IDT) and is pivotal in identifying the physical, psychological, social and spiritual needs of the patient and family. He/she, in working with the RN case manager, initiates the appropriate interventions in support of the patient and family. The LPN also provides a continuous appropriate, comprehensive and responsive plan of care. Works under the supervision of a Registered Nurse.


Job Description:

  • Follow the plan of care of patient/family care and services.
  • Outlines admission plan of care with the IDT and RN Case Manager.
  • Provides nursing care as determined by the plan of care and the RN Case Manager.
  • Educates patient/family and other primary caregivers, in appropriate care techniques.
  • Serves as the patient/family advocate by maintaining regular communication with ALF staff, long term care facility staff, and RN Manager.
  • May participate in providing bereavement support to families and associated bereavement activities as appropriate.
  • Ensure and document appropriate use of medication, durable medical equipment and supplies
  • Participates in team meeting.
  • Provides accurate documentation with visit itineraries to the office. Documentation must be completed according to hospice, state and federal guidelines/standards. Documentation must be prepared and turned in according to these guidelines.
  • Follows RN care plan appropriately/reports to RN case manager on each patient.
  • Monitors the recertification/reauthorization process and provides input for this process.
  • Responds to on-call issues as clinically appropriate
  • Works under the direction of the RN Case manager.
  • Communication Skills Participates in substantive chart reviews as assigned by the Regional Manager.
      • Qualifications:

        • Currently licensed to practice nursing in the state
        • A minimum of two years nursing experience in acute-care hospital nursing in either medical, surgical or oncology unit or other commensurate experience.
