Social Worker – Skilled Care Admissions Coordinator

 

ClarkLindsey, a premier Community serving older adults, located in Urbana, IL, is seeking a licensed, proactive Social Worker to serve as our Skilled Care Admissions Coordinator. This unique dual-role position combines skilled care admissions with psychosocial support, care planning, and resident advocacy for older adults in a skilled nursing environment.

Key Responsibilities

 

Admissions & Care Planning

• Serve as the primary contact for prospective residents and families.

• Manage referrals, pre-admission assessments, and the full admissions process.

• Collaborate with others to create and update resident care plans.

• Participate in interdisciplinary team meetings to review progress and plan discharges.

• Coordinate insurance notifications, appeals, and communication.

• Complete and submit MDS 3.0 assessments.

 

Grievance Officer Designee

• Document and track resident concerns.

• Assist with investigations and ensure confidentiality.

• Ensure timely and fair resolution of grievances.

 

Resident & Family Council Coordination

• Facilitate and document monthly Resident Council meetings.

• Organize quarterly Family Council meetings.

• Serve as liaison for follow-up to concerns.

 

Psychosocial & Mental Health Support

• Conduct assessments and provide short-term counseling.

• Refer residents to external longer-term mental health providers.

• Lead support groups and educational sessions.

 

Resident Services Team Collaboration

• Guide residents and families through care transitions.

• Participate in quality assurance initiatives.

Qualifications

 

Required:

• MSW with LSW or LCSW (Illinois).

• 2+ years in senior living or healthcare.

• Proficiency in MDS 3.0 and Medicare regulations.

• Experience in the responsibilities listed above.

 

Preferred:

• Knowledge of skilled care admissions regulations.

• Experience in fast-paced interdisciplinary settings.

 

Compensation & Benefits

 

• Salary: $64,000–$67,300, based on experience. Bonuses: Up to 2% annually.
