Job Description

Title: Registered Nurse

Location: Clinton Residence

Schedule: 28 hours per week

Hourly Rate: $36.40

 

Program Description:

Clinton Residence is a transitional treatment facility licensed and regulated by the NYS Office of Mental Health. The facility provides transitional housing for 57 formerly homeless, single adults with chronic mental illness, some of whom are dually diagnosed with a substance abuse disorder.

 

Overall Responsibilities:

 RNs provide on-site nursing services under the direction of the Nurse Manager

 

Essential Duties and Responsibilities:      

  • Participate in weekly clinical meetings with medical staff, shelter case management, & administrative staff
  • Perform intakes on all new clients assigned to the program for referral and to connect to care at primary care sites, mental health, & psychiatry services
  • Liaison to connect clients with drug user health services, PREP/PEP, & HIV care management if needed
  • Case management for high-risk clients coordinating care for priority medical needs
  • Assist with prior authorization for medications or medical supplies
  • Assisting with medication refills and attaining new prescriptions
    • Oversight of medication systems, Medication Self-Administration records (MSAR) for clients who self-administer their medications in shelter, including: Assisting clients with pill-packing of their medications
    • Ensuring safe storage of medications
    • Ensuring clients have appropriate supplies and access to medications
    • Monitoring client reactions for side effects and reporting to prescribing provider
    • Maintain records of controlled substances using controlled substance logs
    • Education on medication adherence
  • Fulfill patient specific orders designated by the medical provider
  • Oversight of LPN and coverage as needed for tasks including:
    • Obtaining vital signs
    • Administering medications, injections, and treatments as ordered by medical providers
    • Ordering stock medications, immunizations, and other medical supplies as needed
    • Provide nursing screenings and documentation using nursing templates
    • Perform ordered procedures including, but not limited to, EKG’s, Hearing and Vision tests, Blood Glucose, specimen collection and processing etc.
  • Maintain emergency equipment, AED, oxygen tanks, and other clinical equipment
  • Monitor medication expiration dates
  • Discard discontinued or expired medication
  • Record refrigerator and freezer temperatures using designated datalogger
  • Report on refrigerator excursions or warning alerts
  • Travel to cover locations if needed for coverage
  • Other duties as assigned

 

Qualifications:

  • New York State licensed RN
  • Working with homeless individuals preferred.
  • Must have excellent oral and listening skills. 
  • Must have the ability to interface with clients as well as all levels of staff.
  • Must have knowledge of Microsoft Office, especially Word and Excel. Comfort with electronic medical record documentation required, with knowledge of eClinicalWorks a plus
