Patient Care Liaison

Job Summary:

Sweetwater Care is seeking an experienced Patient Care Liaison/Business Development Professional to drive strategic referral relationships, census growth, and market expansion across our Skilled Nursing Facilities in the Kansas market. As a key member of our team, you will cultivate partnerships with hospitals, physicians, case managers, health plans, and community referral sources, while also supporting mentorship and coaching of business development staff to strengthen overall region performance.

Responsibilities:

• Develop and execute a strategic referral plan to drive census growth and market expansion across our Skilled Nursing Facilities in the Kansas market

• Cultivate and maintain strong relationships with hospitals, physicians, case managers, health plans, and community referral sources to increase referrals and admissions

• Identify and capitalize on opportunities to expand our market presence and increase our competitive advantage

Requirements:

• 2+ years of experience in Hospital or Physician Liaison, Referral Generation, New Business Development, or a related field

• Strong understanding of post-acute care and healthcare sales strategy

• Excellent relationship-building and communication skills

• Ability to work independently and as part of a team

• Strong analytical and problem-solving skills
