Wellness Coordinator- Full Time


We are seeking a compassionate and driven Licensed Practical Nurse (LPN) to serve as our Wellness Coordinator.

In this role, you will lead, develop, and empower our dedicated team to deliver exceptional care to our Assisted Living residents.


Job duties include but are not limited to:

  • Participate in the hiring process of hospitality-oriented resident care staff and Licensed Practical Nurses
  • Create a weekly schedule for the care staff, ensuring that all shifts are filled.
  • Train and supervise staff to maintain a highly functioning team environment.
  • Review job performance of care staff to ensure that our residents are receiving the best service possible.
  • Develop care plans for residents.
  • Facilitate communication with residents and family members.
  • Other duties as assigned


Qualifications:

  • Ideal candidates must possess strong relationship building skills.
  • Minimum of 2 years of management experience with evidence of being a strong leader
  • Experience in training, stating expectations for employees and measuring their progress.
  • Current Florida Nursing license
  • Familiar with ACHA regulatory requirements for Assisted Living Facilities

Basic Functions:

To coordinate the administrative and health care needs for a defined resident population. To plan and coordinate nursing care to residents requiring long-term or rehabilitative care. To ensure an effective and efficient environment for residents, family and staff.


Essential Functions:

  • Works using the guidelines established by Department of Community Health and company policies and procedures.
  • Assesses, plans and evaluates nursing care delivered to residents requiring long-term or rehabilitation care.
  • Implements the resident plan of care and evaluates the resident response.
  • Directs and supervises care given by nursing personnel.
  • Provides input in the formulation and evaluation of standards of care.
  • Maintains knowledge of necessary documentation requirements.
  • Maintains knowledge of equipment set-up, maintenance and use.
  • Maintains confidentiality and resident rights, regarding all resident /personnel information.
  • Provides resident/family/caregiver education as directed.
  • Conducts self in a professional manner in compliance with community policies.
  • Initiates emergency support measure (i.e., CPR, protects residents from injury)


Minimum Qualifications:

  • Graduation from a basic educational program in professional nursing or related field.
  • A minimum of one-year experience in an acute care facility setting preferred.


Specific Requirements:

  • Must be able to read, write, speak and understand the English language.
  • Must possess the ability to make independent decisions when necessary.
  • Must be able to relate information concerning a resident’s condition.
