Key Responsibilities:
- Develop and implement individualized treatment plans to improve residents' mobility, strength, and overall physical function.
- Perform comprehensive evaluations and assessments to identify physical therapy needs and goals.
- Collaborate with interdisciplinary teams, including occupational therapists, speech therapists, nurses, and physicians, to ensure holistic care.
- Provide education and training to residents, families, and caregivers on therapy techniques and strategies for optimal recovery.
- Maintain accurate and timely documentation of therapy sessions, progress notes, and discharge plans in compliance with regulatory standards.
- Utilize evidence-based practices to deliver effective rehabilitation services.
- Foster a supportive and motivating environment that encourages residents to achieve their highest potential.
- Participate in team meetings, quality improvement initiatives, and ongoing professional development activities.
Qualifications:
- Licensed Physical Therapist (PT) in CA (or eligibility for licensure).
- Doctorate in Physical Therapy from an accredited program.
- Experience in skilled nursing, rehabilitation, or geriatric care preferred but not required.
- Strong interpersonal, communication, and teamwork skills.
- Commitment to providing compassionate, patient-centered care.
- Knowledge of relevant documentation and compliance standards, including Medicare regulations.
