Hospice Chaplain – Full Time

Serenity Hospice Care is seeking a compassionate and dedicated Hospice Chaplain to join our interdisciplinary team in Berks County, Pennsylvania. This role provides meaningful spiritual and emotional support to patients and families during end-of-life care.

Position Overview

The Hospice Chaplain is responsible for providing spiritual care, counseling, and emotional support consistent with each patient’s beliefs, values, and wishes. You will work closely with the hospice care team to ensure holistic, patient-centered support for both patients and their loved ones.

Key Responsibilities

• Provide spiritual and emotional support to patients, families, and caregivers

• Conduct spiritual assessments and develop individualized care plans

• Offer prayer, counseling, and grief support as appropriate to patient preference

• Collaborate with nurses, social workers, and interdisciplinary team members

• Support families through anticipatory grief and bereavement

• Document visits and care provided in accordance with hospice guidelines

Qualifications

• Experience in hospice, hospital, or pastoral care preferred

• Ordination or endorsement from a recognized faith tradition preferred

• Strong communication and interpersonal skills

• Ability to provide compassionate, non-judgmental care to individuals of all backgrounds

• Valid driver’s license and reliable transportation required
