Part Time Home Care Caregiver Coach

Position Summary: The Caregiver Coach provides guidance, education, and support to primary caregivers of clients enrolled in the SFC/ATTC Medicaid Waiver Programs. This role is responsible for assessing the needs of both the participant and caregiver, developing individualized coaching plans, and delivering ongoing training and support.

The Caregiver Coach monitors progress, conducts regular reassessments, and adjusts coaching strategies as needed. This position also develops and delivers relevant training materials, resources, and tools to support caregivers in effectively managing participant care and maintaining their own well-being.

At a minimum, the Caregiver Coach engages with caregivers on a bi-quarterly basis to assess evolving needs and provide strategies related to participant care, behavior management, and caregiver self-care. Visit frequency may be adjusted based on program requirements.

Essential Duties:

Caregiver Assessment & Coaching

  • Conduct initial consultations to assess caregiver needs, goals, values, strengths, and areas for development
  • Develop and implement individualized caregiver coaching plans
  • Provide ongoing education and structured coaching based on assessment outcomes
  • Perform annual caregiver competency assessments, as required

Service Delivery

  • Deliver coaching services in-home, virtually, or telephonically using HIPAA-compliant communication platforms
  • Ensure a minimum of two (2) in-person visits per quarter (or at least once every six weeks), in accordance with state requirements (subject to change)
  • Provide additional support through secure electronic communication tools to ensure accessibility and responsiveness

Caregiver Support & Training

  • Educate caregivers on:

    • Infection prevention and control practices in the home
    • Managing participant anxiety and crisis-related concerns
    • Behavior management and identifying triggering events
    • Effective verbal and nonverbal communication techniques
    • Strategies for managing challenging behaviors
    • Home safety and risk prevention
  • Support caregivers in applying stress reduction techniques and reducing isolation
  • Assist in developing crisis management, emergency, and back-up care plans

Care Coordination & Documentation

  • Document all coaching activities and relevant updates in the caregiver�s electronic record following visits
  • Communicate significant changes, concerns, or risks to the Service Coordinator in a timely manner
  • Update client service plans as needed, at minimum annually

Engagement & Monitoring

  • Maintain consistent engagement with caregivers (minimum bi-quarterly) to monitor progress and evolving needs
  • Adjust coaching strategies and tools based on ongoing assessments
  • Perform other duties as assigned

Position Requirements & Competencies:

  • Must have high school diploma or equivalent.Bachelor�s Degree preferred
  • Experience in home care, case management, behavioral health, or direct support
  • Minimum 1 year of experience working with caregivers, individuals with disabilities, or aging populations
  • Experience working with Medicaid Waiver programs (SFC/ATTC preferred)
  • Excellent oral and written communication skills
  • Ability to multi-task under tight deadlines
  • Strong communication skills and interpersonal skills
  • Valid driver�s license and proof of insurance is required
