Position Summary
The Social Worker provides psychosocial support to residents and families, assists with care planning, and collaborates with the interdisciplinary team to support emotional well-being and quality of life.
Primary Responsibilities
Resident and Family Support
Conduct interviews to assess psychosocial needs
Provide counseling and emotional support to residents and families
Offer grief and loss support during transitions and changes in care
Care Planning and Resources
Develop individualized care plans in collaboration with the care team
Assist residents and families with accessing social services and community resources
Crisis Intervention and Support
Provide crisis intervention services as needed
Offer supportive counseling to address emotional and psychological concerns
Communication and Collaboration
Maintain clear communication between residents, families, and staff
Collaborate with nursing and interdisciplinary team members to ensure coordinated care
Qualifications
Bachelor’s degree in Social Work or related field (Master’s preferred)
Previous experience in a healthcare or rehabilitation setting preferred
Strong communication and interpersonal skills
Compassionate, resident-centered approach
Ability to maintain confidentiality and handle sensitive information
