Position Summary

The Social Worker provides psychosocial support to residents and families, assists with care planning, and collaborates with the interdisciplinary team to support emotional well-being and quality of life.

Primary Responsibilities

Resident and Family Support

  • Conduct interviews to assess psychosocial needs

  • Provide counseling and emotional support to residents and families

  • Offer grief and loss support during transitions and changes in care

Care Planning and Resources

  • Develop individualized care plans in collaboration with the care team

  • Assist residents and families with accessing social services and community resources

Crisis Intervention and Support

  • Provide crisis intervention services as needed

  • Offer supportive counseling to address emotional and psychological concerns

Communication and Collaboration

  • Maintain clear communication between residents, families, and staff

  • Collaborate with nursing and interdisciplinary team members to ensure coordinated care

Qualifications

  • Bachelor’s degree in Social Work or related field (Master’s preferred)

  • Previous experience in a healthcare or rehabilitation setting preferred

  • Strong communication and interpersonal skills

  • Compassionate, resident-centered approach

  • Ability to maintain confidentiality and handle sensitive information
