Regional Social Worker

Job Summary

Allaire Health Services is seeking a compassionate and experienced Regional Social Worker to join our team. Responsibilities:

• Provide social work services to patients and residents across our facilities, including residential, long-term, rehabilitation, and subacute care settings

• Conduct assessments, develop care plans, and provide counseling to patients and families to address social, emotional, and psychological needs

• Collaborate with interdisciplinary team members to develop and implement individualized care plans

• Provide education and support to patients and families on disease management, coping strategies, and community resources

• Facilitate discharge planning and arrange for post-discharge services as needed

• Maintain accurate records and reports, adhering to all regulatory and compliance requirements

• Participate in quality improvement initiatives and contribute to the development of policies and procedures

Requirements:

• Bachelor's or Master's degree in Social Work (MSW) from an accredited program

• Minimum of 3 years of experience as a social worker in a nursing home

• Licensure or certification as a Social Worker in the state of New Jersey (LSW or LCSW)

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment and prioritize tasks

• Proficiency in Electronic Medical Records (EMRs) and other software applications
