Job Summary
Silvis Center Rehab and Care, a compassionate and patient-centered healthcare organization, is seeking a dedicated and skilled Licensed Practical Nurse LPN to join our team in Silvis, Illinois. Responsibilities:
• Provide direct patient care, including administering medications, wound care, and mobility assistance
• Develop and implement individualized care plans, in collaboration with the healthcare team
• Monitor patient progress, report changes, and adjust care plans as needed
• Assist with vital signs, vital functions, and basic medical procedures
• Maintain accurate and detailed records of patient interactions and care
• Collaborate with other healthcare professionals to ensure comprehensive care
• Participate in educational programs and training to stay current with industry developments
Requirements:
• Current LPN licensure in the state of Illinois
• Ability to work varied shifts, including days, evenings, and weekends
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Compassion, empathy, and a passion for delivering high-quality patient care
