What You’ll Do:

• Evaluate and treat residents to improve mobility, strength, and overall function

• Develop individualized care plans tailored to each patient’s goals

• Collaborate with an interdisciplinary team to ensure the best outcomes

• Complete timely documentation using Net Health and PCC

What We’re Looking For:

• Licensed Physical Therapist (PT)

• SNF/LTC experience preferred, but not required

• Strong clinical and communication skills

• A motivated, patient-centered approach
