Our agency will employ direct care staff that are certified nursing
assistants at our PHCP agency. Our direct care staff will take care of
individuals’ ADLs, household management, community-based shopping and
appointments; working closely with family members and others to support
Clients’ progress towards better health and well-being. All certified nursing
assistant staff report directly to our Administrator, and are supervised and
monitored by our qualified RN.
Duties and Responsibilities:
1. Assist
residents with all activities of daily living, including showering, dressing, grooming,
toileting, mobility, feeding, taking medications,
cleaning, and other duties as requested.
2. Assist
residents with managing home, activities; following goals on their service care
plan.
3. Assist
individuals with self-administering medications.
4. Respond
promptly to individuals’ requests for assistance.
5. Accurately
document provision of care as directed.
6. Stay current
with changes in care/support (e.g. medications, treatments, behavior plans,
etc.).
7. Alert program
management in the event of an individual’s change in status or behavior.
8. Assist with
weighing individuals, and gathers and records other data as requested.
9. Complete all
documentation and reports within program timelines.
10. Actively participates in resident, family, team and
staff meetings.
11. Attend trainings as assigned maintain certification
of required trainings (CPR, First Aid, and Medication).
12. May possibly transport Clients as request in their
own vehicle or staff’s own personal vehicle.
Qualifications:
- A high school diploma or equivalent. One year experience providing direct care
preferably to individuals - Ability to understand, speak, read and write
fluent English; sensitive to client’s needs. - Strongly prefer that a candidate will have a
demonstrated understanding of and competence in serving culturally diverse
populations, can communicate effectively, use good judgment, high energy level
, function well in a team atmosphere; perform basic computer functions. - Must be able to perform each essential duty satisfactorily.
- Must have a valid driver’s license and access to
an operational and insured vehicle in addition to willingness to use it to
transport residents.
